Medicare GUIDE Dementia Support Across the Austin Area
Families caring for someone with dementia often need two kinds of help. They need a clinical team to coordinate the care plan, and they need reliable support at home so the plan works in daily life. The Medicare GUIDE model is designed to connect those pieces.

Qualicare Home Care Greater Austin works with participating GUIDE organizations to provide authorized respite and practical support in the home. When respite is approved through the program, Qualicare Home Care is paid by the GUIDE organization. The eligible family is not charged for that authorized GUIDE respite service.
What the GUIDE Model Provides
GUIDE stands for Guiding an Improved Dementia Experience. The Centers for Medicare and Medicaid Services created the model to improve dementia care, reduce strain on unpaid caregivers and help people remain safer in their homes and communities.
CMS currently allows qualifying respite services up to an annual program limit that is adjusted over time. The GUIDE participant determines eligibility, care tier, authorization and available respite amount.
A care plan only helps when it can be carried into the ordinary moments of the day.
The GUIDE Model Includes:
- A comprehensive assessment and person centered care plan
- Care coordination and access to a care navigator
- Education and support for the unpaid caregiver
- Access to a support line through the GUIDE participant
- Respite services for eligible participants, subject to program authorization
How Qualicare Home Care Helps at Home
Our care professionals support routines, personal care, meals, safety, companionship and meaningful activity. We do not diagnose dementia or replace the medical professionals managing treatment. Our role is to help the person and family live the plan at home.
Routines & Personal Care
Support with daily routines and personal care so the care plan works in the ordinary moments of the day.
Meals, Safety & Meaningful Activity
Help with meals, a safer home and activities that matter to the person, not just supervision.
Calm Communication & Redirection
Our care professionals use calm communication and redirection when someone is confused, anxious or resistant.
Dementia Care at Home
Consistent companionship and practical dementia support at home, whether or not GUIDE covers the amount of help needed.
Communication With Family & Care Team
We communicate observations to the family and, when authorized, to the appropriate care team.
Authorized GUIDE Respite
When respite is approved through the program, the GUIDE organization pays Qualicare Home Care and the eligible family is not charged for that authorized service.
Respite That Gives the Family Caregiver Real Time Back
Respite can allow a spouse, daughter, son or other unpaid caregiver to:
- Sleep
- Attend an appointment
- Work or travel briefly
- Spend time with the person they love without managing every task
It can also give the person living with dementia consistent activity and companionship rather than simply supervision. Our team learns the person before planning the visit. A familiar routine, preferred music, time outside, a favorite meal or a meaningful task can make the hours feel normal and worthwhile.
Who May Qualify
A participating GUIDE organization makes the final eligibility decision. Families should complete screening with a GUIDE participant rather than assume they do or do not qualify.

Talk to a Care ExpertFrequently Asked Questions
Is GUIDE the same as ordinary Medicare home health?
No. GUIDE is a dementia care model that provides care coordination, caregiver support and qualifying respite. Medicare home health is a separate benefit with different eligibility and covered skilled services.
Does the family pay Qualicare Home Care for GUIDE respite?
When Qualicare Home Care provides respite that has been authorized through a participating GUIDE organization, that organization pays Qualicare Home Care and the eligible family is not charged for that authorized respite service. Services outside the GUIDE authorization may be arranged separately.
How quickly can GUIDE enrollment begin?
Enrollment timing belongs to the participating GUIDE organization and depends on screening, diagnosis documentation and assessment. Qualicare Home Care can explain its own assessment and staffing timeline separately so families know what to expect.
Can we use Qualicare Home Care if we do not qualify for GUIDE?
Yes. Families may arrange private pay dementia support, respite and personal assistance even if GUIDE is not available or does not cover the amount of help needed.
