How We Approach Care Planning in Victoria
Our care planning begins with a simple phone call. Once you speak with one of our local Care Experts, we will set up an assessment meeting where we can go into more detail about your loved one’s needs and your overall goals for their care. Then, we’ll apply our 360° Approach to make sure nothing is missed in the care planning process.
Through our 360° Approach, we consider the following four areas that play a key role in comprehensive care:
- Daily Tasks & Household Management. Does your loved one need help with daily tasks at home? We can assist with bathing, dressing, preparing meals, and other activities.
- Specialized Health Care. Is your family member returning home from the hospital or do they have an ongoing medical condition? We offer specialized services, like Dementia Care & Alzheimer’s Care, for clients with various illnesses and injuries, provided by trained professionals.
- Companionship & Outings. We offer companionship, mindful activities, special outings, and other services to promote the emotional and mental well-being of our clients.
- Family & Care Coordination. Caring for a loved one is a family affair, so we offer specific support for families including respite care and assistance for family caregivers, transparent communication with other family members, and help with the logistics of care.
