Mobility Aid and Home Equipment:
Based on the client’s needs, we assess and prepare for any mobility aids or home equipment.
Medical Appointments:
Assistance with scheduling and managing ongoing medical check-ups and appointments.
Personalized Care Plan:
Our RNs create a tailored care plan for at-home care, ensuring all needs are addressed.
Discharge Day Support:
On the day of discharge, our RN will meet the client at their home to ensure a smooth transition.
Ongoing Assessments:
